Frequently Asked Questions

Frequently Asked Questions
How is Frontline Health Advocates different from my primary care provider?
Frontline Health Advocates (FHA) is a faith-based ministry that provides ministerial member support and advocacy assistance to members navigating immunization requirements and accommodation concerns involving bodily autonomy. Through its private, closed, members-only ministerial network, FHA assists members in understanding applicable requirements, organizing relevant information, coordinating communications, and navigating available accommodation processes through our resource referral service.
What is an immunization accommodation?
An immunization accommodation is a request for an alternative arrangement when an individual’s medical circumstances may make an otherwise applicable immunization requirement inappropriate or unsafe. FHA assists members in navigating the accommodation process. Member health professionals independently evaluate each member’s individual medical circumstances and determine, within their professional scope and clinical judgment, what documentation or recommendations may be appropriate.
How is accommodation documentation different from a standard doctor’s note?
A standard doctor’s note may briefly describe a medical condition or recommendation. Accommodation documentation may provide additional information regarding the member’s individual medical circumstances and the basis for requesting an accommodation.
Does FHA provide state-specific immunization exemptions?
FHA does not issue state-specific immunization exemptions. State exemption processes are separate legal and administrative mechanisms governed by applicable state requirements. FHA assists members in navigating available accommodation processes, including processes that may involve federal disability-protection requirements. Whether a request is accepted depends on the applicable requirements and the decision of the relevant institution or agency.
Are member health professionals licensed?
FHA’s private professional network includes attorneys, physicians, nurses, nurse practitioners, physician assistants, and other qualified health professionals who may review member intake information and consult with members, consistent with their professional scope, training, and clinical judgment. Each member’s documentation is reviewed and signed by a physician, and includes an attorney review and signature in connection with the legal or accommodation-related aspects of the document.
Where may accommodation documentation be used?
Members may pursue accommodation requests in a variety of settings, including employment, education, childcare, travel, immigration-related processes, healthcare access, public accommodations, and other institutional or government settings. Requirements and procedures vary by setting. Institutions and other covered entities remain responsible for complying with applicable federal civil-rights and disability-accommodation laws. FHA cannot guarantee how an independent institution, agency, or other third party will administer or comply with those legal requirements. FHA works with members and member health professionals to help ensure that accommodation documentation is individualized, professionally supported, and responsive to the requirements applicable to the member’s circumstances.
What services are included with FHA membership?
Annual membership provides access to FHA’s ministry programs, including ministerial member support, advocacy assistance, member health professional coordination, and access to FHA’s private, closed, members-only network of health professional resources.
How much does member health professional consultation or documentation cost?
Any fees for health professional consultation, evaluation, or documentation are communicated to members before services are provided. Fees may vary depending on the services requested and the provider involved.
How long does the process take?
Processing time depends on the member’s circumstances, the availability of the member health professional, the information needed for evaluation, and the requirements of the requesting institution or agency. On average, the process takes eight business days to complete.
How long is accommodation documentation valid?
Accommodation documentation is typically valid for one year, although its validity and continuing usefulness depend on the member’s circumstances, the provider’s professional judgment, and the requirements of the institution or agency reviewing the request. A requesting institution or agency may require updated information or additional documentation.
How do I meet with a member health professional?
When consultation is appropriate, members receive instructions for meeting with a member network health professional. Consultations take place by phone. Members should provide accurate contact information and be available at the scheduled consultation time.
Do I need to submit medical records?
Members should provide complete and accurate information when completing the patient intake form requested by the health professional.
What if my school, employer, or organization requires its own form?
Members may provide organization-specific forms to FHA for review and coordination with the member health professional when appropriate. FHA will determine whether the form can be completed and what information is appropriate to provide. Additional fees apply for administrative or provider services.
What if I change my mind?
Members should review the applicable service terms regarding cancellation. When cancellation is permitted, members must cancel before the applicable service or provider consultation occurs.
What should I do if my accommodation request is denied?
Members may contact FHA for ministerial member support and advocacy assistance related to the accommodation process. FHA may help members organize relevant information, communicate with the appropriate institution or agency, and identify available resources within FHA’s private, members-only ministerial network. FHA cannot guarantee a particular outcome.
Does FHA provide legal advice or legal representation?
FHA does not provide legal advice or legal representation. Members may be directed to appropriate legal or professional resources within FHA’s private, members-only ministerial network when a matter falls outside FHA’s role. Any legal services are provided directly by an independent professional under a separate agreement.
Can FHA guarantee that an accommodation will be honored?
No. FHA does not control the decisions or actions of independent healthcare providers, employers, schools, agencies, government entities, travel providers, or other institutions. Each institution or agency is responsible for reviewing and deciding whether to approve, modify, or deny an accommodation request.
Are refunds available if an accommodation request is denied?
